CO2 Fractional Laser Before and After: What Real Results Look Like

The banner above is an illustration of the kind of change CO2 fractional laser aims for. It is not a photo of a real patient or a real treatment result.

Most people searching for CO2 fractional laser before and after photos want to know one thing: will my skin actually look different, and how long will it take? This post explains what a genuine before and after should show for acne scars, texture and stretch marks, how many sessions it usually takes to get there, and where the laser works well and where it does not.

Reviewed by Dr. Swati Aggarwal (PGDHA, Facial Aesthetics; Advanced Aesthetic Injector), Director of Facethetics Beauty Clinic, 10+ years in medical aesthetics.

How to read CO2 laser before and after photos

Before you look at any clinic's photos, including ours, check three things:

  • When the "after" photo was taken. A photo taken one week after a session shows peeling and fresh pink skin, not the final result. Collagen keeps building for three to six months, so an honest "after" photo is taken at least six to eight weeks after the last session.
  • How many sessions it took. One dramatic photo after six sessions tells you very little if you are expecting results from one.
  • Same light, same angle. Harsh overhead light makes scars look deeper. Soft front light hides them. If the lighting changes between the two photos, the comparison is not fair.

One more warning sign: if the before and after look identical except for perfectly smooth skin, with the same stray hairs, same expression and same shadows, the after photo has probably been edited. Real skin after CO2 laser looks better, but it still looks like skin.

CO2 fractional laser before and after: acne scars

Acne scars are the most common reason people come to us for fractional CO2. Rolling scars (wide, soft-edged dips) and boxcar scars (flat-bottomed with sharper edges) respond best, because the laser resurfaces the skin around them and the new collagen lifts the base of the scar. Narrow, deep ice-pick scars improve less with laser alone.

In a genuine before and after for acne scars, look at the cheeks in side lighting. The dips should look shallower, scar edges softer, and the surface should catch fewer shadows. Redness and active pimples are a different problem, and laser photos that mainly show clearer, less red skin are not showing scar improvement.

What patients usually notice first is that makeup sits flatter and the skin looks less uneven in side lighting. Scars rarely disappear completely. A realistic goal is that they become noticeably shallower and less visible from a normal talking distance.

Fractional CO2 laser resurfacing before and after: texture, pores and fine lines

Resurfacing is where fractional CO2 often shows visible change in fewer sessions. Rough patches get smoother, enlarged pores look tighter, and fine lines around the eyes and mouth soften as collagen rebuilds.

In before and after photos for resurfacing, the change shows up as smaller-looking pores on the nose and cheeks, smoother skin that reflects light more evenly, and softer lines at the outer corners of the eyes. Deep wrinkles and sagging change much less, because they are caused by volume loss and laxity, not the surface.

Texture and fine-line concerns often settle in two or three sessions. The skin also tends to look brighter once healing is complete, because the dull outer layer has been replaced.

One thing fractional CO2 is not right for is melasma or general pigmentation. Heat-based resurfacing can make melasma worse. If pigmentation is your main concern, Q-switch laser, Cosmelan or Dermamelan or chemical peels are usually the better choice.

Fractional CO2 laser for stretch marks: before and after

Stretch marks are where we set expectations most carefully. Fractional CO2 can improve their texture, make them look flatter, and help them blend closer to the surrounding skin colour. It does not erase them.

How much they improve depends mostly on their age:

  • Newer stretch marks (still pink, red or purple) respond better, because the skin is still actively remodelling.
  • Older stretch marks (white or silvery, often years old) change less. The goal here is a smoother surface, not a colour change.

A realistic stretch mark before and after shows lines that are narrower, flatter to the touch and closer to the surrounding skin tone. If a photo shows stretch marks completely gone, be sceptical.

Stretch marks usually need more sessions than facial texture, and the improvement is gradual. If you are looking for a dramatic change, a consultation will tell you honestly whether laser is worth it for your marks.

What your skin looks like between "before" and "after"

Before and after photos skip the middle. It starts with the session itself: numbing cream for 45 to 60 minutes, eye shields on, then the handpiece moves over the skin in small sections. The red dot you see in the clip below is the aiming beam that shows where the next pass will land.

Fractional CO2 laser pass along the temple at Facethetics Beauty, Delhi. The patient wears eye shields for the whole session.

Here is what the first few weeks look like after a session:

  • Day 1 to 2: redness like a strong sunburn, warmth and some swelling, often most noticeable under the eyes on the first morning.
  • Day 3 to 5: the skin darkens slightly and feels rough, like fine sandpaper, as tiny treated dots form micro-crusts. Do not scrub or pick them.
  • Day 5 to 7: flaking and light peeling. Most people are comfortable going back to work around this point.
  • Week 2 to 4: pinkness fades gradually. The skin looks fresher, but this is not the final result yet.
  • Month 2 to 6: collagen keeps remodelling under the surface. This is when scars and texture show the most improvement, which is when an honest "after" photo should be taken.

Plan your sessions around this. Avoid booking one in the two weeks before a wedding or a big event.

How many sessions before you see a difference?

Some people see smoother skin after the first session, but the before and after results you see online are almost always from a course of treatment. As a general guide:

  • Texture, pores and fine lines: often 2 to 3 sessions
  • Mild to moderate acne scars: usually 3 to 5 sessions
  • Deep or long-standing scars and stretch marks: can need more, sometimes combined with other treatments

Sessions are spaced 4 to 6 weeks apart, so the skin has fully healed and collagen has had time to build before the next pass. At Facethetics, a session costs ₹5,000 for the face or ₹6,500 for the face and neck. The fractional CO2 laser treatment page has the full cost breakdown and what happens during a session.

Why results on Indian skin depend on the settings

Most Indian skin is medium to darker in tone, and that makes it more prone to post-inflammatory hyperpigmentation, which is temporary darkening after the skin heals. It is the side effect we watch for most closely, and it is also the main reason some before and after photos online show a patchy "after".

Three things keep the risk low: laser density and energy set for your skin tone and not just your scar depth, waiting if you have a fresh tan, and strict sun protection before and after each session. A slightly gentler session repeated is safer than one aggressive pass, and it still gets you to a good result.

Who should not have CO2 fractional laser right now

  • Pregnancy or breastfeeding
  • Active acne breakouts, cold sores or any skin infection in the treatment area
  • Recent or current isotretinoin (Accutane-type) medicine. The doctor decides the safe gap.
  • A history of keloids or thick, raised scarring
  • Active melasma in the area
  • A fresh tan, or heavy sun exposure planned in the weeks after

CO2 Fractional Laser Before and After: Common Questions

How soon after CO2 fractional laser will I see results?

The skin looks fresher once peeling finishes, around one to two weeks after a session. The real change in scars and texture shows over the next three to six months, as new collagen builds under the surface.

Are CO2 laser results permanent?

Scar improvement from collagen remodelling is long-lasting. Your skin will still age naturally, and new acne can cause new scars, so good skincare and sun protection help you keep the result.

Can fractional CO2 laser remove acne scars completely?

Rarely. Most patients see their scars become noticeably shallower and less visible, but complete removal is uncommon. Anyone promising 100% removal is overselling.

Does fractional CO2 laser work on old white stretch marks?

It can make old stretch marks smoother and flatter, but the colour changes less than with newer red or purple marks. Expect improvement, not disappearance.

Why do some before and after photos look worse or patchy?

Usually because the photo was taken too early, during the healing phase, or because the skin developed temporary darkening after treatment. On Indian skin, settings matched to your skin tone and strict sun protection reduce that risk.
